Create or log into your account to schedule an onsite health screening and complete your health assessment, or call the StayWell Helpline if you do not have access to a computer. All required activities must be completed and submitted to StayWell before October 11, 2019. For more information, visit wellwisconsin.staywell.com or call the StayWell HelpLine at 800-821-6591.
