New voluntary Self-Identification of Disability Form in MyUW
Recently, the federal government revised the Voluntary Self-Identification of Disability Form. The new form provides an expanded list of examples of disabilities and updates the preferred language for disabilities. We encourage all employees to review the new form and self-report or update their personal information accordingly. Please note that self-identifying as an individual with disabilities is a separate process from requesting an accommodation. By completing this self-identification form, you are not requesting a reasonable accommodation.
Your information can be updated at any time by using the Personal Information application in MyUW at my.wisc.edu. Information is treated as confidential and will not be kept in employment files/records. Submission of this information is voluntary and helps ensure that UW–Madison has the most up-to-date and accurate information for government reporting and assessment of workforce diversity efforts.
